You can use the tool below to help you effectively analyze your thoughts and actions that are causing your depression. You can use the tool daily to learn to control and change your cognitive distortions and rid yourself from depression.
First I provide a chart of the cognitive distortions and their meanings.
Secondly I provide an example of how to use the form.
Lastly I provide the form for you to use on a daily basis to help you learn to deal with your depression

The form below is designed for you to use immediately when you find yourself feeling depressed, anxious, angry etc. It will help you begin to recognize and thus change the situations, thoughts and sensations that are causing your depression or unpleasant feeling.

The form below is designed to help you become positive when two very common cognitive distortions or ANT's (Automatic Negative Thoughts) that perpetuate and sustain depression creep in; Discounting the Positive and Seeing the Glass Have Full. Learning to recognize and combat these thoughts will give you some very needed tools has you combat and overcome your depression.
The form below is designed for you to recognize all the great, positive attributes and gifts you possess. And yes, all of us have positive qualities that we currently have or have exhibited in the past. When we are depressed it's hard to see any positive now or in the past, but in order to learn how to deal with and change your depression (And yes, you have the power to change your depression!!), you must learn to change your automatic though patterns. So, use the form to begin your journey of being positive and combatting depression.
Once you complete the form, simply repeat your I AM statements at least twice daily. Or come back to this site twice daily and fill in the form. The act of typing your I AM statements allows you to own your statements and declare that you are positive. Repeat your I AM statements every day until they become daily, Automatic Positive thoughts.
